THE ACCURACY OF COLPOSCOPY DIRECTED CERVICAL BIOPSY; ACCORDING TO AGE GROUPS AND GRADE OF CERVICAL INTRAEPITHELIAL NEOPLASIA
 
Background: The accuracy of colposcopy has been questioned in some recent studies. The aim of the study was to to evaluate the overall perfect agreement, underestimation and overestimation rate between colposcopically directed biopsies and the definitive excisional specimens in different age groups. Methods: In a retrospective study of 456 patients who underwent conization within 6 months after colposcopy directed cervical biopsy were included. The most severe pathological result for both cervical biopsy and conization specimen were used for comparison. The patients were divided into three groups according to their age: under 30 years, between 30-50 years of age and over 50 years of age. The overall perfect agreement, underestimation and overestimation rates were compared among these age groups. Results Overall perfect agreement, underestimation and overestimation rates between pathological results of colposcopy directed biopsies and cervical conization were 50.4%, 16.0% and 33.6% respectively. Overall perfect agement for CIN1, CIN2 and CIN3/CIS/AIS were 55.2%, 27.6%, 71.7% respectively. Overall perfect agreement rates of under 30 years of age, between 30-50 years of age and above 50 years of age were 54.2%, 52.8%, 35.9% respectively. There was lower overall perfect agreement rate of in above 50 years of age group. Conclusion Our results confirm that the safety of colposcopy is less reliable in women older than 50 years. It has a relatively high accuracy for the diagnosis of cervical CIN3/CIS/AIS and MIC lessions.
